Our Outreach Coordinator can assist with the following
The Frederica Senior Center Outreach Department is available to meet with seniors or their families in the office or by appointment in their homes to assess needs, discuss services and programs available in the community, provide advocacy and support and make referrals. The Outreach Worker can assist in a crisis.
Our center offers transportation, hot home cooked meals, activities, day trips, card club, bingo, out of state trips.
The staff maintains strict confidentiality and no information shared with the Outreach Worker will be released to anyone without permission from the elder, including family members. The Outreach Worker is able to coordinate services with other agencies such as Public Health, VA, Hospital Social Workers, Nursing Homes, Rehab, Community Action, Meals on Wheels, Food Pantry, Financial, State assistance, Food Stamps, Fuel Assistance, Medical Bills and many other organizations as well as from families and elders themselves.
Outreach also has monthly healthcare screening and yearly health fairs. Outreach has guest speakers for topics on Medicare, Social Security, Private health insurance providers, Prescription assistance, Transportation.
